Adolescent Prevention Intervention Specialist

We are currently seeking to fill a vacancy in our Prevention Services Department. The Prevention Intervention Specialist for the Adolescent program works with very high risk adolescents in small groups or one-on-one to assist them with behavior changes.  Youth may be gang involved, using drugs/alcohol, teen parents, and/or engaging in other high risk behaviors. Group programs are in community and/or alternative school settings, and may utilize technology as a prevention tool by helping youth create videos, games, music, etc. with prevention messages. The Adolescent Prevention Intervention Specialist also provides HIV test counseling and STD/hepatitis C screening in community settings.

The position requirements are:

  • Minimum of an Associates degree (Human Services or related area; Bachelors degree preferred)
  • One year experience working with the target populations (please outline this experience in your cover letter)
  • Good group facilitation skills in such areas as communication, decision-making, and healthy relationships
  • Ability to provide HIV, STD, and related information to the target populations
  • Must have independent means of transportation and ability to work a flexible schedule, including early evening hours
Music, videography and/or game making technology skills a plus.

Salary: $15.38 an hour

Intensive Case Management

We are seeking experienced case managers for our Albany & Schenectady offices. Our Intensive Case Manager provides intensive and comprehensive case management to Medicaid enrolled persons with HIV disease and collaterals in order to ensure access to services necessary to maintain their quality of life and promote independent functioning.

Duties include:

  • Observe and enforce HIV Confidentiality Law.
  • Perform case management activities to ensure optimal functioning of client and collaterals. Such activities include: intake, assessment, service plan development, implementation, monitoring and follow-up, crisis intervention, advocacy, interagency and intra-agency coordination and maintenance of client records.
  • Maintain all required contacts with clients and service providers. Client contact includes home visits, transporting clients to needed appointments, and/or meeting clients at designated service providers to provide support and advocacy.
  • Make necessary referrals and assist with intake process as necessary.
  • Coordinate and ensure the utilization of appropriate and needed services as identified in the Service Plan.
  • Assess needs, refer, advocate for, and ensure utilization of appropriate services for collaterals in conjunction with client’s Service Plan.
  • Complete required documentation by program deadlines.
  • Provide information about HIV disease, medications, psychosocial issues and harm reduction education.
  • Provide coordination of all services (medical, legal, housing, substance abuse, mental health) for client to include case conferencing.

Only applicants that meet the following minimum qualifications need to apply.

Minimum qualifications include Bachelor ’s degree in health, human or education services and one year of qualifying experience* or associate’s degree in health or human services or an LPN and two years of qualifying experience.*

*Qualifying experience means verifiable full- or part-time case management or case work with the following populations: persons with HIV infection, and/ or persons with a history of mental illness, homelessness, or chemical dependence. Experience with families preferred.

Knowledge/skills required:

  • Familiarity with and sensitivity to specialized needs of people with HIV;people with substance abuse and mental health issues;people of color; GLBT community.--Familiarity with and ability to access community resources and services.
  • Strong verbal and written communication skills, and empathetic and active listening skills.
  • Attention to detail.
  • Strong organizational skills including Time Management and ability to prioritize.
  • Ability to work as a member of a team.
  • Ability to provide strength-based, client centered, proactive case management.

Salary: $15.65+ hour (depending upon level of experience)
