Case Manager - Ryan White program

Albany office

This position will provide case management, intake, assessment and referral services to HIV infected individuals to increase and ensure access to and retention of medical care necessary to maintain their quality of life and promote independent functioning.

Duties include :

  • Incorporate best practices and abide by Article 27-F HIV Confidentiality Law.
  • Perform case management activities which include intake, assessment, service plan development, implementation, referrals, monitoring and follow-up, crisis intervention, advocacy, interagency and intra-agency coordination and maintenance of client records.
  • Conduct home visits.
  • Coordinate transportation and provide support services as needed that affect the HIV-related clinical status of a person with HIV/AIDS.
  • Case conference with other agencies and AIDS Council staff regarding mutual clients on a regular basis.  Develop linkages with other programs and organizations that serve both directly and indirectly people with HIV infection.
  • Provide referrals to appropriate services and assist with the intake process as necessary.
  • Advocate for client’s needs both internally and with other providers. Attend appointments with clients, as needed, to increase access to medical care and provide advocacy.
  • Ensure required documentation and case management tasks (including intakes, assessments, reassessments, and service plans) are completed by required due dates.
  • Provide information, education and support to clients regarding HIV disease and available HIV-related resources and services.

Qualifications:

  • Bachelor’s Degree and one year case management experience with target population or one year human services experience or Associate’s Degree and two years case management experience with target population
  • Familiarity with and ability to access community resources and services.
  • Strong communication skills, including verbal ability, writing style and active listening skills.
  • Strong organizational skills including time management and ability to prioritize; attention to detail.
  • Ability to provide strength-based, client-centered, proactive case management.
  • Must have independent means of transportation for travel throughout catchment area.

Salary: $26,500-$28,000

Prevention Projects Manager -

Project HOPE (Albany office)

This position will be responsible for the oversight, development and coordination of comprehensive, culturally competent prevention interventions with a primary focus on men who have sex with men (MSM) with services inclusive of the GLBT community throughout our 15 county catchment area. Responsibilities include but are not limited to:

  • Oversee, develop and coordinate a continuum of project activities including:
    • comprehensive group and individual level behavior change interventions
    • HIV  and STD counseling and testing, including phlebotomy services, in accordance with NYS regulations
    • Online interventions
    • Outreach
    • Peer volunteer activities
    • Pubic awareness media campaigns
  • Provide supervision to program staff
  • Collaborate with community partners to build capacity and project sustainability
  • Complete and oversee maintenance of documentation and reporting
  • Conduct quality assurance and evaluation of project deliverables

Minimum qualifications:

  • Bachelor’s degree; Master’s preferred
  • Two years post-baccalaureate experience in public health, education, human services or related field
  • Two years direct supervisory/management experience
  • Extensive knowledge of the target population*

Knowledge, Skills and Abilities required:

    • Sensitivity and extensive knowledge of substance abuse, HIV/AIDS, hepatitis and related issues.
    • Clear, coherent writing style and good public speaking ability.
    • Well-organized, accurate and attentive to detail.
    • Knowledge of behavior change theory, group dynamics, evaluation methodologies and curriculum.
    • Ability to interact with people from diverse backgrounds within a climate of mutual respect, inclusion, enrichment, and growth.
    • Must have independent means of transportation for travel throughout a 15 county region

Substance Abuse-Case Manager

Albany office

The position is responsible for providing case management services to active substance users in order to facilitate access to drug treatment and/or relapse prevention services.

Duties include:

  • Observe and abide by all applicable laws.
  • Develop and provide transitional case management activities to active substance users in order to facilitate access to drug treatment and/or relapse prevention services.  Responsibilities include facilitating referrals, assistance obtaining necessary documentation, referral follow-up, etc.
  • Transport clients to needed appointments and/or meet clients at designated service providers to provide support and advocacy.
  • Develop and implement strategies to engage hard-to-reach populations to include enhanced outreach, health communication and individual level interventions.
  • As applicable, conduct HIV counseling and testing, STD and Hepatitis screening in venues serving target communities. 
  • Network with community service providers serving the target population to identify needs, promote program services and generate referrals, and expand access to resources within the communities served.
  • Complete documentation in compliance with reporting requirements.

Qualifications:

  • Associate’s Degree with experience working with targeted populations (HIV/AIDS and substance abuse); Bachelor’s Degree preferred
  • Attention to detail; well organized
  • Good oral and written communication skills
  • Ability to interact with people from diverse backgrounds
  • Ability to work early morning and occasional evenings
  • Must have independent means of transportation to travel to designated program sites, per program needs
  • Must have current unrestricted driver’s license and proof of automobile liability coverage of $100,000/$300,000 to transport clients
  • Bilingual (Spanish) a plus

Salary: $28,000-$30,000

Supportive Services Specialist - Schenectady office

The Supportive Services Specialist will provide group and individual level supportive services, including psychosocial support, treatment education, and health education/risk reduction to HIV infected individuals to ensure access to treatment and care leading to improved clinical outcomes.

Duties include:

  • Incorporate best practices regarding confidentiality into all job duties and abide by Article 27-F, HIV Confidentiality Law.
  • Provide individual and group level supportive services (both online and in-person) including psychosocial support, treatment education, and health education/risk reduction services.
  • Develop curricula and outlines for supportive services.
  • Conduct brief intake and assessment of client needs. Provide monitoring and follow up as needed.  Provide referrals to appropriate services.
  • Maintain client records per requirements.
  • Case conference with other agencies and AIDS Council staff regarding mutual clients on a regular basis.  Develop linkages with other programs and organizations that serve both directly and indirectly people with HIV infection.
  • Ensure required documentation are completed by due dates as required.
  • Provide information, education and support to clients regarding HIV disease and available HIV-related resources and services.

The following positions are available as an expansion of our Project HOPE services to MSM and the GLBT community.

Prevention Intervention Specialist (3 positions available)

The Prevention Intervention Specialist positions will provide comprehensive, culturally competent prevention interventions with a primary focus on men who have sex with men (MSM) with services inclusive of the GLBT community.  All positions provide HIV and STD counseling and testing (including phlebotomy services in accordance with NYS regulations), online interventions, outreach, and individualized risk reduction counseling in community-based settings. 

Each of the vacancies also represents a particular focus and geographic area:

  • Capital District based position - Group, peer leadership model with young MSM in community, school and college campus settings. 
  • Albany-based position with a regional approach - Community awareness and outreach to the GLBT community in a fifteen county region.  Areas of specialization include Prevention with HIV-infected MSM and their partners; public awareness campaigns to reduce stigma and increase testing for syphilis.
  • Plattsburgh based position** (part-time 21 hours per week) - Primarily providing HIV/STD testing to young MSM in community, school and college campus settings.  Additional outreach and community engagement activities to support project goals.  

Salary: $15.38 hour/$28,000 annualized

Minimum requirements for all positions:

  • Associates degree in Human Services or related area; Bachelors degree preferred.
  • One year experience working with the target populations. 
  • Ability to provide HIV, STD, and related information to the target populations.
  • Must have independent means of transportation and ability to work a flexible schedule, including evenings and weekends.
