Supportive Housing Case Manager works together with the client to obtain and/or maintain housing Case Manager assists the client with:

• Housing stabilization
• Development of independent living skills
• Budget counseling
• Landlord negotiation
• Locating and maintaining available housing and financial resources
• Referral to community resources
• Assistance and follow-up services provided for 1 year
