case manager

Our case managers provide intake, assessment, case management and referral services to HIV infected individuals to increase and ensure access to and retention in medical care necessary to maintain their quality of life and promote independent functioning.

Major Activities:

  • Observe and abide by the HIV Confidentiality Law.
  • Case management activities include: intake, assessment, service plan development, implementation, referrals, monitoring and follow-up, crisis intervention, advocacy, interagency and intra-agency coordination and maintenance of client records.
  • Responsible for conducting home visits, coordinating transportation and completing emergency financial requests as needed.
  • Case conference with other agencies and AIDS Council staff regarding mutual clients on a regular basis.
  • Develop linkages with other programs and organizations that serve both directly and indirectly people with HIV infection.
  • Attend appointments with clients, as needed, to increase access to medical care and provide advocacy.
  • Ensure required documentation are completed by due dates as required by funding sources.
  • Provide information, education and support to clients regarding HIV disease and available HIV-related resources and services.

Qualifications:

  • Bachelor’s Degree and one year case management experience or Associate’s Degree and two years case management
  • Familiarity with and sensitivity to specialized needs of people with HIV, substance abuse and mental health issues, people of color, gay, lesbian and transgender individuals
  • Familiarity with and ability to access community resources and services
  • Strong communication skills, including verbal ability, writing style and active listening skills
  • Attention to detail; strong organizational skills including time management and ability to prioritize
  • Ability to provide strength-based, client-centered, proactive case management
  • Must have a valid driver’s license and independent means of transportation for travel throughout catchment area
